Altum Health's Musculoskeletal Model of Care

Rapid Access to Specialty MSK Treatment

Altum Health's new Musculoskeletal (MSK) Model of Care provides doctors and their patients with rapid access to UHN's specialty MSK services, enabling patients to get the appropriate care & attention throughout all phases of their treatment.

How Does It Work?

When a patient is referred to Altum Health, all relevant medical information is carefully reviewed and the patient will undergo a Clinical Screen, at no cost to them, by a Registered Physiotherapist with advanced training in musculoskeletal issues.

The Clinical Screen will determine 1 of 4 courses of action:

  1. 1) Home exercise program – education and instruction on home exercises & important self-management techniques
  2. 2) Rehabilitation program – short course of physical rehabilitation
  3. 3) Secondary assessment – further assessment by a physician with MSK expertise, which may include MRI, CT or blood work if needed
  4. 4) Surgical consult – a timely appointment will be arranged to see an orthopaedic surgeon

It is our goal and mission that patients receive access to the right care at the right time.

About Musculoskeletal Injuries & Diseases

  • Musculoskeletal (MSK) injuries and diseases affect 11 million Canadians each year
  • The number of Canadians with MSK issues is predicted to increase with the aging baby boomer population
  • MSK conditions affect the body's muscles, bones, nerves, tendons and ligaments
  • Some examples of MSK conditions include carpal tunnel syndrome, osteoporosis, arthritis, tendonitis and back pain
  • Musculoskeletal disorders are the most common type of workplace injury
  • Early intervention is the key to preventing MSK disorders and long-term disability

Why a New MSK Model of Care?

  • The target wait time in Ontario to see an orthopaedic specialist is 182 days, but currently, the actual wait time can take up to 305 days
  • Many patients waiting to see an orthopaedic surgeon end up not being surgical candidates and their MSK condition(s) could have been better managed with early intervention
  • Due to long wait times to see the wrong specialist and duplicate referrals back and forth, most patients don't receive timely access to the right treatment options
  • Altum Health's MSK Model of Care links the patient to our integrated network of MSK experts so they can get the right care, right away.
